Test Ordering Process: Ontario

Requisitions

  • Patient specimens must be accompanied by a fully completed, signed requisition;
  • The Ontario Ministry of Health and Long-Term Care Laboratory Services Requisition Form (available in the Downloadable Forms section of this web site) must be used for OHIP-funded tests;
  • Patient specimens to be tested by Public Health Laboratories (PHL) must be accompanied by a fully completed and signed Ontario Agency for Health Protection and Promotion (OAHPP) Public Health Laboratories requisition (available in the Downloadable Forms section of this web site);
  • The names on the requisition and the specimen container must match exactly; any unlabelled specimens received will not be processed;
  • Please use the exact test name terminology listed in the Test Information section of this web site (or in your copy of the Gamma-Dynacare Directory of Client Services manual) to avoid delays in processing;
  • Any additional information about a patient that could be helpful to both the laboratory and the clinician can be communicated on the form under “Additional Clinical Information”;
  • All cytology and histology samples must be accompanied by a completed cytology or histology requisition, available in the Downloadable Forms section of this web site;
  • The requisition must be dated and signed by the clinician or designate. If a rubber stamp is used, initials are required.

Repeat testing

Repeat testing for individual patients who return to the same Patient Services Centre each time can be requested for a period of up to six months without further documentation. The initial signed requisition must indicate the frequency of testing required. The information will be kept on file at the Patient Services Centre.

“To follow” testing

Occasionally patients will be required to collect specimens at home in addition to the specimens collected by Gamma-Dynacare. These "to follow" tests will be honoured by Gamma-Dynacare provided that the patient returns to the laboratory within 30 days of the date on the original requisition. If the patient does not return within 30 days, they will be asked to return to your office for another signed requisition.
 

Test Ordering Process: Quebec

Requisitions

  • All tests requested by a licensed physician require a fully completed, signed requisition each time laboratory testing is required;
  • Gamma-Dynacare’s standard bilingual requisition is available to all physicians throughout the Province of Quebec, and can be obtained by contacting our Main Laboratory at (514) 486-2299 or 1-800-697-3831, #1;
  • Gamma-Dynacare accepts all types of medical laboratory testing requisitions;
  • Should you require a copy of the test results be transmitted to another physician, you must indicate this on the requisition;
  • Requisitions specific to our Ultrasound Services are also available by contacting our Main Laboratory.
