Patient specimens must be accompanied by a signed requisition. The
names on the requisition and the specimen container must match exactly;
any unlabelled specimens received will not be processed. Please use
the exact test name terminology listed in the Test
Directory (or in your copy of the Gamma-Dynacare Directory
of Client Services manual) to avoid delays in processing.
Please complete all designated information areas on the Ministry of Health and
Long-Term Care Laboratory Services Requisition and Claim Form. Any additional
information about a patient that could be helpful to both the laboratory and
the clinician can be communicated on the form under Additional Clinical
Information.
Note: All cytology and histology samples must be accompanied by a completed
cytology or histology requisition.
Repeat testing for individual patients who return to the same Patient Services
Centre each time can be requested for a period of up to six months without
further documentation. The initial signed requisition must indicate the
frequency of testing required. The information will be kept on file at the
Patient Services Centre.
Occasionally patients will be required to collect specimens at home in addition
to the specimens collected by Gamma-Dynacare. These "to follow" tests will be
honored by Gamma-Dynacare provided that the patient returns to the laboratory
within 30 days of the date on the original requisition. If the patient does
not return within 30 days, they will be asked to return to your office for
another signed requisition.
Please note that the Ministry of Health and Long-Term Care does not recognize
profile testing. All tests must be ordered individually and by name. Terms such
as lytes and hep screen are not acceptable and need
clarification. Use of such terms can delay testing and require your signature
again. Verbal requests may be accommodated only as an exception to written
requisitions; however, all verbal requests will generate a requisition that
will require the signature of the clinician. The requisition must be dated and
signed by the clinician or designate. If a rubber stamp is used, initials are
required. Requisitions can be customized to suit specific requirements of
government, industry and business.
