While most routine medical laboratory tests are covered by provincial health
insurance plans such as OHIP, some important newer tests are not. These tests,
such as homocysteine and blood allergy testing (see
Test Descriptions), might be covered by supplementary health insurance
as part of a patient’s employment benefits package. Because each insurance
carrier has its own criteria for deciding which tests to cover, patients are
advised to consult their benefits plan or to contact a representative from
their human resources department for clarification. If a test is not covered by
provincial or supplementary health insurance, patients pay for the test
themselves.
For patient convenience, a downloadable generic
Insurance Claim Form has been included on this Web site. You can either
print the form to share with patients or direct them to the Patients area of
this Web site.
Billing Information
Patients are required to pay directly for all tests not covered by their
provincial health insurance plan and should then seek reimbursement from their
supplementary health insurance provider.
